S: On back perch, started flying around and vocalizing when I entered. Vocalizations hoarse and raspy. Respirations heavy for catch. Hopped up to food platform then cross perch when placed in F6. Flew to high front perch when approached to drop food.

O: Caught for weigh and moved to F6. Feet good. Portal wound on L patagium clear of scabbing, feathers starting to grow back in area. Dropped 66g rat, rab, chx with meds on food
Terbinafine: 0.36 cc (50 mg/ml compounded ) PO

A: Respirations seem improved since last catch. Respiratory rate appeared normal when at rest, both before and after catch: no visible or audible breathing when on perch or flying between perches when not evading catch. Increased respiratory rate and audible rasping only present when started getting stressed for catch, respiratory rate settled as was in hand. Raspy respirations seemed less wet then they were at last catch. L patagial wound continuing to heal

P: Weigh again in 2 weeks and consider moving to large flight. Continue to monitor respirations. Monitor flight in larger space.

A: BAR. Edges and margins of hole in patagium have healed very well and didn't see any signs of swelling, irritation/inflammation, etc. Feathers have began to grow back. Flight is erratic, but it's difficult to fully assess it in this size cage. I don't think she should go to a large flight yet, however if we can move her to a slightly larger cage, I think it could be beneficial.

S: Audible wheezy breathing heard from bird prior to catch. Vocalizations very weak. Bird seems to be flying decently in small flight.

O: Per KK, breathing/vocalizations slightly better in hospital than they were prior to catch. Vocalizations still raspy/weaker than normal, harsh audible breathing/wheezing heard without stethoscope, wheezes and harsh lung sounds heard on auscultation. Drew blood from the R medial metatarsal vein. Removed sutures and scabbing from wound hole in patagium - small amount of blood seen after scab removal but overall the area looks very healthy. VD/lateral x-rays taken: L ulna fracture unchanged, well healed with small synostosis. Rocks in GI tract, crop looks like it may have a small amount of material in it, GI tract still filling most of caudal coelom and compressing some of lung space. On the lateral view, the distal tracheal looks like it might have some compression from opacities in the cranial thorax- could be possible plaques.
Terbinafine: 0.37 cc (50 mg/ml compounded ) PO

A: Skin looks like it is still doing well, unable to fully assess flight in small cage but no obvious deficits. New onset of respiratory signs today - not sure if bird was interrupted trying to cast or eat when was interrupted by handler, concerned for possible tracheal compression on x-rays that could indicate something like aspergillosis.

P: D/c clindamycin, rx terb, monitor respirations, prioritize reading CBC.

O: Small amount of dried blood on the dorsal humerus sutures and along the sutures in the patagium. Small amount of dehiscence at the cranial aspect of patagium sutures. Cleaned sutures and applied TAO. Performed PT on wing, able to get more extension with a few rounds of PT but still not quite to full extension in carpus. Took x-rays: GI tract still looks very full/backed up, but bird is casting and defecating normally, just very obese.

O: L patagium sutures appear mostly intact with very small opening on cranial aspect of porthole. Dorsal humerus sutures intact with piece of scabbing stuck to sutures. Cleaned suture sites gently w/ chlorohex and applied TAO. Performed gentle PT to L wing, noted less than full extension of wing. Gave oral meds. No leftovers. Dropped 90g of rabbit/chick.

A: L patagium sutures had scabbing a minor bleeding while cleaning. Was not able to pull off scabbing stuck to L dorsal humerus without pulling on sutures, so tried to soak and wipe off as much as possible. Will need to continue to soak scab until it can be fully wiped off. Unsure if there was a suture placed in the cranial aspect of patagium porthole site, but will continue to monitor small opening to ensure it doesn't worsen. Overall sutures are in place and holding. Bird has less extension at elbow and carpal joints. BAR.
Weight change: +18 g (2 %)

S: Jumpy and vocal for catch.

O: See surgery report for details. Removed all infectious tissue, down to the skin on the patagium in order to close skin. Soft tissue swelling noted on the dorsal humerus under the feathers, excised and found infectious material. Cleaned out and closed skin. Bird recovered uneventfully from anesthesia. Took x-rays with bird under GA: bit has excessive amounts of fat, GI tract is extremely full and backed up to the point where the lungs are slightly compressed. Lung sounds were harsh on auscultation. Fracture site is well healed but there is a synostosis between the r/u and the spacing between the r/u near the carpus looks smaller than normal.
Food: SF 83g venison/chick
Enrofloxacin (Baytril): 0.17 cc (100 mg/ml) PO
Meloxicam 0.5 mg-kg: 0.27 cc (1.5 mg/ml compounded) PO

A: Porthole left is patagium after removal of infectious tissue but birds often are still able to fly with these, and they also tend to heal down on their own. Found a granuloma on the dorsal aspect of the L humerus which would be affecting WBC count as well as was able to completely excise. Synostosis may prevent bird from being able to regain full flight. Wing was slightly less than full extension.

P: Switch from enrofloxacin to SID clindamycin. Switch to EOD catch. Restart PT on L wing with catches, monitor sutures on patagium and L dorsal humerus. Monitor for cast, repeat GI x-rays next week.

Surgery report
Procedure - Wound debridement
Performed by - AD
Surgery time - 40 minutes

Pre-medicated with torb IM.  Anesthetized via isoflurane facemask,
the intubated with ET tube.  Took x-rays prior to procedure.
 Plucked and scrubbed the dorsal and ventral aspects of the L
wing - noted a soft tissue mass on the dorsal aspect of the mid
humerus. Started with bird in dorsal recumbency.  Placed lidocaine
into wound site on L wing.  Excised all abnormal tissue, down
to the skin, and incised the skin on the dorsal aspect of the
wing.  Closed skin from the dorsal aspect of the wing to the
skin on the ventral aspect of the wing in order to full close
and cover muscle and other tissues.  Used 4-0 webcryl in a simple
interrupted pattern.  Porthole left in the center of the patagium
after closure.  Turned bird onto sternal recumbency, incised
over the swelling on the dorsal humerus.  Abscess material present
under the skin, removed all and flushed area.  Closed skin with
4-0 webcryl in a simple continous pattern. Performed PT on wing
as bird started to recover - tight extension in the carpus noted.
 Bird recovered uneventfully from anesthesia.

A: BAR. Edges of L wing wound are less pale, but inside of wound is still pale white of dead tissue.

A: BAR. So far, closure of the wing wound seems successful. I'm hesitant to push PT too until that skin heals over more, as there's risk of tearing the sutures out. Will want to continue monitoring carefully.

O: See surgery report for details. Took x-rays with bird under GA: radio-opaque mass on R side of crop, unchanged from last set of x-rays. Debrided wound on L wing, left open to try to heal by second intention. Incised over mass on R side of crop and removed small amount of abscess material, cleaned and closed with sutures. Patient recovered uneventfully from anesthesia.

A: Debrided infectious material from over L wing. Left open to heal and monitor better for reoccurance of infection. Removed abscess from R side of crop.

P: Apply Dermagel to L wing SID. Monitor sutures on R side of crop. Switch from clindamycin to Clavamox for soft tissue infection as bone is looking healthier on x-rays, restart meloxicam for inflammation.

Procedure - Wound debridement

Surgery time - 30 minutes

Pre-medicated with torb IM, anesthetized via isoflurane facemask,
then intubated with 3.0 uncuffed ET tube.  Took x-rays: lots
of soft tissue trauma along L patagium, callus formation starting
at L ulna fracture site, gap still present where fracture has
not fully healed.  Soft tissue lesion along R side of neck unchanged.
 Scubbed ventral aspect of L wing, debrided and removed old sutures
and white/infectious material.  Scrubbed and flushed area, then
flushed site with baytril.  Left open.  Cleaned R side of neck,
incised over firm white lesion with a #15 blade - dissected out
a small abscess from te SQ face over the crop.  Removed and then
closed SQ and skin layer with 4-0 webcryl in separate layers.
 Bird recovered uneventfully from anesthesia.  Covered open wound
with dermagel and left uncovered.

mattress sutures with 4-0 webcryl. Unable to close distal most

skin, at risk for tearing or failing. Placed tissue glue over
sutures. Covered wing with fabric wrap.  Bird recovered uneventfully

A: Debrided dead skin/scabbing on wing. Tissue underneath looks healthy but will need to let heal by second intention as there was not enough healthy skin to close site with sutures. Concerned for appearance of bone in x-rays but bird was recently changed antibiotics and x-rays images lack behind clinical picture so will give more time and recheck x-rays and blood next week.

Procedure - Wound debridement

Surgery time - 25 minutes

Pre-medicated with torb IM, then anesthetized via isoflurane
with facemask. Intubated with 3.0 uncuffed ET tube.  Took x-rays
of L ulna - some healing starting but concerned for bone infection.
 Scrubbed dorsal and ventral aspect of wing.  Placed bird in
dorsal recumbency, debrided scab and white tissue underneath
along the ventral patagium.  Tissue along the humerus looks good,
skin healed so removed sutures.  Small tendon exposure post debridement
along the lateral apsect of the humerus, closed skin with 4-0
webcryl.  Unable to close more skin over open wound, covered
with dermagel and telfa/tegaderm.  Flipped bird over onto ventral
recumbency, removed scabbing and sutures over the dorsal ulna.
 Tissue underneath looks to be healing, covered area with dermagel.
 Placed fabric wrap on wing for overnight, bird recovered uneventfully
from anesthesia.

O: See surgery report for details. Debrided wounds on ventral and dorsal aspect of the L ulna and the ventral L humerus, removed pellet along the L humerus. Placed IM-ESF tie in to stabilize the L ulna fracture. Removed shards of loose, severely damaged bone and flushed wounds thoroughly prior to closure. Placed TAO, telfa and tegaderm over sutured area, placed figure-8 wrap on wing for overnight.

A: Debrided and repaired L ulna fracture - prognosis very guarded due to extent of damage from the bullet to the bone, at risk of nonunion if too much bone was damaged. There is also severe soft tissue injury present from the bullet as well.

P: Change wing wrap and bandages tom.

Procedure - Fracture repair - ulna
Performed by - AD
Surgery time - 60 minutes

Pre-medicated with 2 mg/kg torb IM. Anesthetized via isoflurane
facemask with 3.0 uncuffed ET tube.  Removed wing wrap and took
repeat x-rays: distal L r/u looks improved from intake x-rays,
spacing looks more normal.  Comminuted fragments of ulna fracture
look more spread out.  Plucked and scrubbed the ventral and dorsal
aspects of the L ulna and humerus and the dorsal ulna.  Started
with bird in dorsal recumbency - cleaned the 2 wounds over the
humerus, removed the metal pellet.  Debrided skin edges and closed
with 4-0 webcryl in a simple interrupted pattern.  Some white
infectious plaques present over the ventral patagium, removed
as much as possible.  Cleaned wound site on the ventral ulna,
flushed thoroughly, removed pieces of dead bone and closed area
in the same manner as the humerus.  Turned bird into sternal
recumbency, debrided wound on the dorsal aspect of the ulna,
removed blood clots and dead bone and flushed thoroughly.  Placed
IM pin in the ulna near the elbow and drove normograde across
the fracture site to seat near the carpus.  Took intraop x-rays
to confirm placement - large gap now present at the fracture
site from removing the damaged bone pieces.  Closed wound in
the same manner as the ventral side, placed 2 threaded cross
pins, one in each fragment.  Bent the IM pin around the dorsal
aspect of the wing and secured together with epoxy.  Placed TAO,
telfa and tegaderm on sutured areas on the ulna and humerus,
placed figure-8 wrap on bird.  Bird recovered uneventfully from
anesthesia.

O: Removed figure-8 wrap from the L wing: large amount of bruising/swelling with a hematoma on the L ulna, open wounds on the dorsal aspect of the ulna, the ventral aspect and the ventral humerus. L ulna fracture very unstable, bird very painful even after giving 0.12 cc torb IM. Cleaned wounds and covered with duoderm gel, telfa and tegaderm, placed figure-8 wrap back on bird. When reviewing x-rays, concerned for decreased spacing between the distal L radius and ulna - difficult to tell if there is any crepitus here due to large amount of instability in the ulna in general.

O: Collected blood from R medial metatarsal. Collected fecal from cage. Removed L figure 8 and bandages. Cleaned L ulna/humerus wounds w/ chlorohex and applied honey/duoderm gel, covered w/ telfa/tegaderm. Applied L figure 8. Gave fluids and oral meds. Sprayed w/ Adams. Dropped 35g of chick.

Radiographic findings: L comminuted ulna fx. Gunshot and pellet stuck next to L humerus.
Initial treatment: 0.13cc torb, 0.11cc Simbadol, 46cc LRS SQ, 0.22cc Meloxicam, 0.13cc Baytril, 1cc Calcium Slurry, 0.2cc Pentoxifylline, 1 tablet 62.5mg Clavamox.

A: Patient suffering from L ulna fracture caused by gunshot wound. Metal pellet is stuck underneath skin over L humerus. Guarded prognosis as L ulna fracture is comminuted into multiple fragments, which could make surgical repair difficult. Gunshot wounds also present over fracture site and entry site at L humerus. Wounds appear fresh and L dorsal ulna wound had blood present and not much clotting yet. Also concerned that L distal ulna is rotated at the wrist due to fracture, which may indicate that surgical repair will be needed to keep bone aligned while fracture heals. Patient is otherwise very alert and active during rescue and exam.

P: Rx meloxicam, calcium slurry, baytril, Clavamox, pentox and fluids. SF in AM. Change L wing bandages SID until wounds have no more drainage, clean w/ chlorohex and apply duoderm gel. Collect blood and fecal 9/6. Collect OU IOPs for study. Monitor L figure 8 wrap. HF in PM 9/7 if not eating on own yet in prep for potential sugery.
